Care Planning

The Registered Manager or a senior member of the care team will conduct a “needs assessment” for each prospective resident and upon admission; the information collected will be used to develop a care plan, including any risks associated with daily living.

The care plan shall be developed in consultation with the resident and with their permission, a family member/representative, GP and any other stakeholder deemed necessary.

Care plans are continuously reviewed to ensure appropriate care is being delivered and the residents’ daily needs are being met. Should it become necessary to make amendments to the care plan, staff will always seek the resident’s and/or relatives’ opinion and input, to ensure they are satisfied with the care being delivered.
